Sources of information used for determining a disability and\/or accommodations may include a student\u2019s self-report, direct observation and interaction with the student and\/or documentation from qualified evaluators or professionals.<\/p>\n<p>Documentation may be submitted in, but is not limited to, one of the following formats:<\/p>\n<ul>\n<li><a class=\"pdf\" href=\"https:\/\/www.gcc.mass.edu\/ds\/files\/2022\/12\/Disability-Verification-Disclosure.pdf\">Disability Verification Disclosure<\/a><\/li>\n<li>Qualified provider\u2019s professional letterhead<\/li>\n<li>Official visit summary, such as emergency department discharge papers<\/li>\n<li>Neuropsychological assessment<\/li>\n<li>Individual Education Plan (IEP)<\/li>\n<li>504 Plan<\/li>\n<li>Psycho-educational evaluation (Learning Disability\/ ADHD\/ TBI)<\/li>\n<li>Psychometric test results from general measures of aptitude and academic achievement (Learning Disability\/ADHD\/TBI)<\/li>\n<li>Audiogram<\/li>\n<li>Vision screening results<\/li>\n<li>Other information from a qualified healthcare provider that can provide insight into the student\u2019s limitations and\/or provide support for any requested accommodations<\/li>\n<\/ul>\n<p>We recommend that third-party documentation:<\/p>\n<ul>\n<li>Be typed on letterhead, dated and signed by a qualified professional<\/li>\n<li>State the disability or disabilities<\/li>\n<li>Describe the impact or symptoms of the disability<\/li>\n<li>If appropriate, discuss the severity and\/or expected progression<\/li>\n<li>If appropriate, list medication side-effects<\/li>\n<li>List current and\/or past accommodations<\/li>\n<li>List any recommended accommodations<\/li>\n<\/ul>\n<p>Please note that the documentation you provide to GCC may not be sufficient to receive accommodations at other postsecondary institutions or for external assessments (e.g., NCLEX, NREMT, etc.).<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Documentation may include assessments, reports and\/or letters from qualified evaluators, professionals or institutions. \u00a0Common sources of documentation are healthcare providers, psychologists and diagnosticians.
