Frequently asked questions / How do I request a medical withdrawal?

If a student is requesting an exception to the withdrawal policy based upon medical reasons, parts 1 and 2 of the Request for Medical Withdrawal form must first be submitted. The student fills out part 1 of the form and returns it to Enrollment Services, room C103. Your provider (e.g. doctor, therapist, psychologist) can fill out either part 2 of the Request for Medical Withdrawal form or write a note on their letterhead. Either the form or the note, along with the provider’s business card, must be returned to Elaine Lapomardo, Dean of Enrollment Services, room C103. When both the student and provider information is received, the request will be reviewed and considered, and the student will be notified via mail of the decision. The decision, along with the original request, will become a part of the student’s record.
