  • Physical exam (within in one year)
  • Respirator Medical Clearance Form
  • Vaccinations
    • COVID
    • Influenza (before Oct 1)
    • Meningococcal ACWY
    • Tetanus, diphtheria, Rubella (MMR)
    • Varicella (Chickenpox)
    • Hepatitis B (either vaccine or positive blood titer)
    • 4-panel urine drug screen (done between 7/15-8/15)
    • Annual TB screen
